The policy file was corrected, yet the assistant still gives the old answer. Should a ticket close when the file changes or when the intended user task receives the correct response? Knowledge maintenance needs closure criteria separating source, index and answer, with ownership of each approval.
A ticket describing an understandable incident
In a fictional sign-installation business, site-preparation policy changes. A new text is approved, but the assistant tells a customer the previous conditions. No model call occurred in this educational example. It identifies evidence needed before attributing the problem to content, indexing or another cause.
Record question, answer, time, channel and necessary context, plus the approved policy reference and effective date. Remove unnecessary information under appropriate authorization. A single faulty message does not justify copying an entire conversation archive to a new contractor.
Content owner and implementer may differ
Name who approves the new preparation policy and its effective date, who updates content or the knowledge connection, and who reviews the ticket outcome. One person may hold multiple roles, but technical confirmation of an upload should not silently approve an unmade business decision.
Where sources conflict, the content owner identifies the valid reference before asking the implementer to make the assistant choose. Without an approved answer, the gap remains a decision issue rather than permission to invent a fact. Record the waiting state and owner instead of closing the ticket nominally.
A correction card from source to outcome
Use this card for important incidents. It is not a universal technical recipe: the implementer explains how each level appears in the actual solution, what is observable and what needs alternative evidence. Retain reviewable references without disclosing unnecessary information.
- Incident: question, context, time and incorrect-answer reference.
- Content decision: approved text, owner, effective date and replaced material.
- Knowledge path: connected source, available version, refresh state and visibility limits.
- Verification: original question, nearby wording and an important unaffected case.
- Closure: each result, evidence, unknowns and acceptance owner or reason for holding.
This checklist does not promise visibility into every product stage. If the solution hides the content version used, document that limitation and propose suitable evidence within scope. Limited visibility alone establishes neither refresh success nor failure.
Refresh follows a path, not a universal timer
Copilot Studio describes scheduled synchronization for some file sources and distinguishes knowledge connection paths. Ask which path is used and its limits instead of assuming a universal schedule or manual-refresh capability. Source readiness does not establish every answer's correctness.
Azure AI Search explains that some deletion-detection approaches can leave orphaned index documents if source content is deleted before its deletion is processed. This is a possible mechanism in its context, not a diagnosis for every assistant. Deleting a source file alone does not establish removal from a different solution.
Verify correction without moving the error
In the sign-installation example, the content owner defines acceptable outcomes for the preparation question, a reworded version and an unrelated service question. A specialist would perform authorized checks with synthetic data and record answer, reference and time. This is a proposed exercise, not a performed trial.
One answer may improve while another still uses old context or a different source. Keep the ticket open or split it explicitly; one successful screenshot should not hide remaining cases. A passing sample also cannot guarantee every future wording receives the same response.
Specify who receives recurrence reports and which details link them to the previous correction. Another policy change creates a new content version with its own decision and effective date; it does not justify erasing what was corrected earlier. A concise record preserves decisions without excessive data retention.
Initial knowledge readiness covers preparation, while assistant proposal evaluation helps define evidence. Bring the ticket card when discussing AI agent services with Nextwave to clarify maintenance responsibility after launch.
